  • Action Tracking: Tracks assigned tasks as a result of incident investigations, ensuring accountability and closure.
  • Correction Loop: If the report is sent back for correction, it returns to the responsible person for necessary changes and then goes back to the department safety officer/HOD (Responsible person) for review.
  • Investigating Team/Person Assignment: If the report is accepted then it assign to an investigating team/person, for further processing.
  • 5 Y Analysis: Incident investigator can also perform the 5Y analysis by providing the following details such as Category, Type, Attachment, and Remark.
  • Progress Tracking: Investigating team/person can update the progress of their assigned tasks, request a revision of the target date if need, or mark the task as completed.
  • Revision Requests: If the Investigating team/person requests a revision of the target date, the form returns to the associated person for accept/reject the revision request additionally responsible person.
  • Completion and Approval: Upon completion of the task/invistigation, the Investigating team/person adds comments and uploads relevant files before submitting the form for approval.
  • Approval Process: The responsible person can either send the form back for correction or approve and close it, adding final comments before closing the incident.
